Insurance Reimbursements and Health Savings Plans
Dr. Kaye does not directly participate with any insurers, Medicare, or Medicaid.
We will provide you with a receipt and the appropriate diagnostic codes and the forms needed to submit the receipt to your insurer for reimbursement.
Dr. Kaye is happy to write you a letter to justify the medical necessity of your treatment to your insurer or Health Savings Plan. You may use your Health Savings Account to pay for your visits if they are not covered by your insurance.
Payments and Deposits
We require payment in full at the time of the visit. We require a 50% deposit for your initial visit to secure your first appointment. You may reschedule this visit if you give us a reasonable notice; if you fail to show up for your first visit, then the deposit is not refundable. When you come to your first visit, the deposit is applied to your initial visit fee. This is necessary to keep our costs down and to provide you with as much undivided attention as you need.
We work for you, not the insurance companies. We cannot guarantee reimbursement by your insurer, however, we have found that PPO plans with out-of-network coverage will reimburse a portion of your visit charges. Most insurances and Medicare and Medicaid pay for blood testing and other lab services in full. If you have a high deductible, we can have your lab testing done at a 75% discount if you pay the lab directly. Prescriptions will be paid as usual according to your insurance plan.
Fees
We schedule an initial 90-minute consultation for all new patients which costs $695.00. At that first visit, we will order appropriate testing which is reviewed at your first follow-up appointment. Subsequent types of visits and the costs will depend on your treatment plan. Â We offer many other types of visits and procedures; please contact the office for more information on the costs of those types of visits.
Additional costs may be incurred for some laboratory testing, some medications, and supplements. Depending on your insurance, some, all, or none of these costs may be reimbursed.
